A complete package supporting the examining doctor has to be prepared by the employer, adjuster, or medical manager. The package must consist of the following:

  • Comprehensive history of the occurrence of the injury
  • Copies of all relevant medical records. Drugs being used, course of treatment, and information of employee’s job attitude and job performance
  • Job description showing education, training, experience, hours of work, interaction with peers, and security requirements
  • Consultation with the examining physician about other details that may be needed for evaluation
  • Details from defense attorney about the needs that have to be addressed
  • Documenting all collected data in a letter to the physician
  • Sending notice to the claimant
